Club Referral Form

Please note that each club lasts for 9 weeks and youth will need support in assuring their availability and transportation so they can stay committed to the 9 weeks of the club!

Once referred, your client will be invited to each of the three club cycles offered per year. Please be aware, that progress notes are only available upon request.

Please notify In My Shoes of any placement changes. Your client is eligible to complete their club cycle even if they are adopted or return home.
